Patient Referral Form

The type of tummy tuck performed varies and depends on the individual patient’s quality and the distribution of their fat. Traditional tummy tuck removed excess skin and fat mainly from the central part of the tummy. Liposuction was not included because of fear of skin loss. Liposuction alone will address the excess fat and improve contour but did little to improve the excess skin and laxity of your abdominal wall. These older procedures often produced an unnatural appearing tummy.

High lateral tension tummy tuck combines liposuction with skin excision to improve the contour of the feminine torso. Liposuction is done to improve the contour of the upper flank. Skin tightening is done by placing more tension on the sides of you tummy to give a better hourglass figure. Your muscles are also tightened to enhance your figure. These features are unique to the High Lateral Tension abdominoplasty technique, and are not as readily achieved (if at all) with traditional abdominoplasty techniques.

Today, for my tummy tucks, I perform a modification of the high lateral tension technique developed by Dr. Ted Lockwood. Liposuction suction is aggressively performed in the lateral and central part of the abdomen to improve the contour. Fat is also directly removed to create a thin tummy. Your stomach muscles are shaped to not only to give you a flatter tummy but better shape on your sides. Finally, skin excision is done to place your scar as low as possible.

I just came back from Las Vegas for a forum presented by LifeCell^TM on the use of Strattice^TM for breast revision surgery following breast augmentation or breast lift procedures. Strattice is an acellular dermis made from bovine tissue.  Alloderm^TM, also made by LifeCell^TM, is the human form of acellular dermis that has been used extensively for breast reconstruction for the last several years.  The advantages of using acellular dermis for breast revision surgery is that the extra tissue can provide more implant coverage as well as providing support.  There are strong evidence that acellular dermis also reduces the recurrence of capsular contraction. Alloderm^TM, the most commonly used acellular dermis, is very expensive when used in cosmetic breast surgery.  Now LifeCell^TM has provided Strattice^TM that provides all the benefits ofAlloderm^TMat a reduced cost.

The primary benefit for acellular dermis is its use in breast revision surgery.  Products like Strattice can increase implant coverage and reduce the incidence of implant visibility or palpability.  Acellular dermis can be used to control implant pocket in case of synmastia (uni-boob), lateral migration (implants falling off the chest wall), or bottomed out implants.  This added tissue might improve the predictability of your results.

Another important indication for the use of acellular dermis is in capsular contraction.  We know that recurrence rate for capsular contracture is significantly higher for patients with previous capsular contracture.  Bridging a product like Strattice has been shown to decrease the recurrence of capsular contracture.

I have had extensive experience with Alloderm for the use in breast reconstruction and abdominal wall reconstruction.  I have used Strattice^TM for abdominal wall reconstruction and have recently started to use it for cosmetic breast surgery. Strattice^TM will significantly improve the results in revision breast surgery.

Breast implant placement for breast augmentation is an important choice usually made by the plastic surgeon.  Your final shape and contour of your breasts can be dramatically different based on the breast implant placement.  Breast implants can be placed on top of the muscle (subgladular) or under the muscle (sub-muscular).  Some variation of the two exists including sub-fascial (directly on top of the muscle) and dual plane (mostly submuscular).  Breast enhancement procedures not only help you fill out your breasts but depending on the placement of the breast implants, can also improve your shape.

In subgladular breast augmentation, the implants are either placed directly under your breast tissue (classic) or on top of your pectoralis muscle but below the muscle fascia (sub-fascial).  In either case, the breast implant will fill out your breast and has a mild lifting effect.  Implants tend to have a more natural motion and will not animate (move) with activity.  Patients tend to have less pain.  The best candidates for subglandular breast augmentation are those with adequate breast tissue.

The breast implants used in subgladular breast enlargement may be visible or palpable especially in patients with thin breast tissue or after pregnancy.  Capsular contracture rates are also higher with implants placed above the muscle.  Because of these issues, non-textured silicone gel implants are the recommended for subgladular breast enhancement.

In submuscular breast augmentation, the implant is placed partially below the pectoralis muscle.  A complete submuscular placement is rare.  The pectoralis muscle provides more soft tissue coverage of the implant decreasing visibility and palpability of the breast implant.  The slope of the breast also appears natural and the implant soft tissue junction is less visualized.  Capsular contracture rates are lower in submuscular breast enlargement.  Either saline or silicone gel breast implants can be used.

Submuscular breast enhancement can increase animation or movements of your breast.  These movements are noticeable in some activity as weight lifting or pulling yourself out of the pool.  There is a mild increase in post -operative discomfort but most heal in similar time.

Patients do have choices about placement of their breast implants for breast enhancement surgery.  I feel that most all patients would do better when implants are placed in the submuscular position.  Most patients seeking breast augmentation lack adequate breast tissue (the reason for the surgery).  Aging and changes with pregnancy can lead to decreased breast tissue especially in the upper part of your breast.  Submuscular placement can protect the upper pole and give a natural slope to your breasts, decrease visibility of the implant, and improve long-term satisfaction.
